This 8-part traditional preprinted laser set includes 2-up forms; Employer Copy A (federal, red scannable), D (file), and 1(state) and Employee Copy B (federal), C (file), and 2 (state). All 8.5" x 11" sheets are printed on 20# laser paper with no side perforation (format changed in 2001). Print each copy as a batch, then separate and collate by employee.
