This 6-part traditional preprinted laser set includes 2-up forms; Employer Copy A (federal, red scannable), D (file), and 1(state) and Employee Copy B (federal), C (file), and 2 (state). All 8.5" x 11" sheets are printed on 20# laser paper with no side perforation (format changed in 2001). Print each copy as a batch, then separate and collate by employee. Order the quantity equal to the number of employees for which you need to file, you'll get all of the necessary forms. Mail employer copies in a batch to the SSA and state.
