Use this blank 2-up perforated paper for printing 1099 MISC, 1099R, 1099 DIV, 1099B forms.  Sheets are 8.5"
x11" and constructed of 20# laser paper with one center perforation without any payee instructions on the bank.  
Order by the number of sheets needed.  For compatible envelopes use DWMR OR DWMRS for self seal.
